The Registered Nurse functioning within the Patient Centered Care Model will assess, plan, implement and evaluate care for a group of patients, collaborate with health care professionals, coordinate ancillary staff, and demonstrate professional growth and development. Must be able to provide appropriate care for patients served according to the unit's scope of practice.
EDUCATION: Graduate of an accredited school of nursing (Diploma, AD, BS, BSN).
EXPERIENCE: 2+ years of Home Care & Hospice experience.
HOURS: Per Diem schedule, will work as needed to do home visits for hospice patients.
