Care Coordinator, RN –

Western Connecticut Medical Group (WCMG), an affiliate of our system, currently has an opportunity for a motivated individual committed to providing superior care.

In this role, you will develop, oversee and monitor care management and coordination activities for the patients of the primary care practice. You will support clinical teams, identify high acuity patient populations and work directly to ensure care coordination. You will collaborate with the Practice Medical Director and Managers to develop service initiatives that best serve the needs of the patient panel and the primary care teams.

In addition, you will support the patient-centered medical home model as well as support the transitions of care between the inpatient and outpatient setting, working with multidisciplinary teams. You will work with high-risk patients and provide primary care for a practice population, chronic disease management counseling and continuous quality improvement. Measuring patient care outcomes and collecting data on the patient population is required and you must illustrate an evidence-based care approach to the primary care setting.

Job Requirements


Education, training, experience:
This position as a Care Coordinator requires an Associate’s degree or certification equivalent, current CT State RN license, and a minimum of three years of experience. BSN and previous experience in Primary Care preferred. The successful candidate for this position will service Primary Care Offices in Danbury and the surrounding communities.
